As comfortable as our waiting room is, we know that patients aren’t exactly eager to spend lots of time in it while waiting to meet with the dentists. To save time, one you book your appointment, our patient forms will be emailed or texted to you so that you can complete them at your convenience. When you arrive in-person, we can expedite your initial check-in process. If you aren’t able to fill out the forms, please let us know.
